THIS NOTICE DESCRIBES THE TYPE OF HEALTH INFORMATION ABOUT YOU THAT MAY USED AND DISCLOSED BY ZONSITE. THIS NOTICE ALSO DESCRIBES HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Introduction

The groundwork in building a healthy relationship between a patient and their health provider begins with the confidence, trust, and security of personal information. Today, the health industry is changing. Now, more than ever, the level of trust built between the health provider and patient is of utmost importance. The Zonsite.com web site ("the Service") provided by Zonsite serves as a link between health providers and patients, and is dedicated to upholding policies over patient privacy while still providing convenient, innovative, and personalized information in a manner that conveniently improves your overall health.

In order to maintain the privacy of confidential health information, federal law requires that health providers, such as physicians, physician's staff, or personal therapists disclose their legal duties and privacy practices with respect to such health information. Some state laws are not as specific as federal law therefore requiring other organizations such as Zonsite, to protect and maintain the privacy of personal health information. Zonsite collects and maintains protected health information on behalf of health care providers, such as your physician and/or therapist and as such is bound by certain duties. In light of this and the importance of privacy information, Zonsite has established this Notice. This Notice provides you information concerning your rights and Zonsite’s responsibilities under the law, as well as Zonsite’s privacy practices. Zonsite is required to abide by the terms of this Notice.

Privacy Policy Overview

The terms of this Notice apply only when you are navigating within the Service and to information collected and maintained by Zonsite, whether Zonsite obtained that information directly from you or Zonsite obtained that information from your physician. You are operating within the Service until you have been notified that you are exiting the Service. Zonsite is not responsible for the content or privacy practices of other web sites. We are not responsible for the information collection practices of the non-Zonsite links that you click from our web pages. We cannot guarantee that these third parties do not use cookies or place on your computer cookies that may identify you personally. In the event that you disclose personal information to other web sites, you are subject to those web sites' privacy policies. We encourage you to review those web sites’ privacy policies before entering any information. In addition, the terms of this Notice apply to physician activity within the service only. The terms of this notice do not apply to the physician and/or therapist’s privacy policies, as they exist within their own scope of practice. You should contact your physician for a copy of his/her overall privacy policy.

Information we collect

Zonsite can receive information about you in two ways. Zonsite can collect certain information about you, via your physician, your physician's staff or your therapist at the web site. Zonsite can also receive information about you directly when you register for the service. Information about you collected by Zonsite will be used for: storing and maintaining your information, assisting your physician and/or therapist in providing treatment to you, and assisting in carrying out his/her health care operations. Unless you exercise the option to prohibit the sharing of information within the Service as described in the privacy policy, all information obtained in connection with the service may be shared within the service to either facilitate your use of the Service or to assist your physician or other health care providers in providing health care and information to you. Information that we obtain solely in connection with the service will not be disclosed to any third party that is not affiliated with Zonsite (other than your physician and/or therapist or his/her agents) unless you explicitly authorize us to do so or unless we are otherwise required to do so by law.

Registration and contact information.

When you register to use the Service, which includes selecting a participating physician or practice, you will be required to provide certain information including: your name, address, email address, phone number, social security number, health insurance information, and other related information.

Health information provided by you or your physician.

In using the Service, you can voluntarily provide your physician, physician's staff and/or therapist with a personal medical history using the Service. This information will be collected and stored by Zonsite on behalf of your physician. Your physician may in turn elect to print and/or store this information in his/her own office. Via the Service, this information will be made available to your physician or your physician's office staff at their discretion.

Cookies.

Zonsite also collects information about your use of its web site through the use of cookies. Cookies are small pieces of information passed by the web server to your browser. We use cookies to identify you to the web server during page requests and to keep your session alive. You can control the use of cookies through your browser settings.

How Zonsite uses your information

Zonsite only uses and discloses your personal information to facilitate your use of the Service. Zonsite may use your information as necessary for its proper management and administration or to carry out its legal responsibilities. Zonsite may also use or disclose your information in carrying out health services (e.g., verifying the status of your insurance coverage) and health care operations (e.g., quality assurance, personnel review).
In addition, collective information about you may be used for statistical analysis. This information is based on the overall population of users of the Service and does not single out any particular individual’s information. Zonsite will not use or disclose your information without your express authorization other than those reasons stated in this Notice. Zonsite or your physician, your physician's staff and/or therapist may use or disclose your information in their attempt to provide you with the best health care possible. Such instances include but are not limited to: appointment reminders sent to personalized e-mail addresses as well as other health related benefits or services. Using the Service, you authorize Zonsite to disclose your information to third parties (e.g., other physicians and their affiliates) to assist such parties in providing services to you. You have the right to terminate such action as stated in this Notice upon written request to Zonsite and its participants (e.g. your physician and his/her affiliates).

Circumstances may occur where Federal law permits or requires uses or disclosures without your permission for purposes beyond those discussed above. Such purposes include but are not necessarily limited to: compliance with other laws; to perform health oversight activities; to comply with a lawful request from a law enforcement agency or with a judicial order or other lawful process; to perform certain limited special government functions; and to comply with applicable worker compensation laws. In most cases, Federal law takes the necessary steps to ensure and safeguard the required uses or disclosures described above.
In the event of a change of ownership and/or approval of licensure, the information collected may be transferred to such party or parties. Such individuals or entities will be required to follow the terms of this Notice in the place of Zonsite's current owners.

Zonsite will take the necessary precautions available in order to ensure the privacy/security of its user’s information. Zonsite, as well as any of its user’s, acknowledge that transmissions made by means of the Internet cannot be made absolutely secure. Zonsite will not be liable for disclosures of your personal information due to errors in transmission or the unauthorized acts of third parties.

User Rights

You have a number of important rights with respect to the information held by Zonsite on behalf of your physician. Zonsite recommends that you consult with your physician, your physician’s staff and/or therapist regarding your rights as a health care consumer. If you require a copy or seek information regarding your personal health information that is stored within the Service, you have the right to obtain a paper/digital copy with your written consent. You may exercise your rights under this paragraph by contacting our Client Support Services, in writing, at support@Zonsite.com.

Information Protection

All information provided to Zonsite is handled securely. Zonsite attempts to keep information from our users as secure as possible against unauthorized access and use. Zonsite will make every precaution necessary to safeguard user provided information in the event that such information be made available to third parties.

Access to your account is controlled by a login ID and a password, which you choose. Strict login ID and password rules virtually eliminate the unauthorized user from gaining access. Zonsite encrypts every password on its database but is not responsible for the mismanagement of such passwords and/or computer use at the user end. Zonsite recommends that its users take precaution with their usernames and passwords.

When you submit personal information to the secure areas of the Zonsite.com web site, this information is encrypted through Zonsite’s “Hosting” company. At present, such encryption involves the use of 128-bit Secure Sockets Layer ("SSL"), an encryption software program.

Children's Privacy

The Service is not intended to be used by children under the age of 18. As such, the service does not collect personal health information from those under the age of 18 yet parents may, at their discretion, provide information about their children. As already stated in the Notice, information provided to Zonsite by parents will be kept safe and secure.

Policy Changes

Zonsite dedicates itself to protecting your privacy. Due to ongoing safety and security standards, Zonsite may periodically change or update this Notice. As changes occur, you will be notified. You may be given a choice as to whether or not we use your information in regard to the changes. By continuing to use this web site, after choice has been given, you agree to be bound by any such revisions and should therefore periodically visit this page of the web site and print the latest version of the Notice for your records. Zonsite will apply the latest version of the Notice to all information that it holds, regardless of when Zonsite came into possession of the information. For your information, the date of the last update to this Notice is stated at the top of the document.
